COPPERSTATE may ask you to provide your personal and/or business information to us when you visit Our Web Site and/or COPPERSTATE may ask you to provide personal and/or business information to us when you send an email to COPPERSTATE through our website. The information that we collect from you is for the purposes of providing you with certain types of insurance which we sell and other services that we render to you concerning your insurance. This information which we may ask you to provide is your name, home address, mailing address, telephone number, email address, and other information, applicable to both you personally and/or your business. This information we call "Personal Information". This Personal Information will be provided to the insurance company or other companies concerning matters on your behalf for which you have made application or inquiry or when we obtain eligibility, apply, enroll, or purchase an insurance policy or other policy on your behalf to establish you as a policy holder. We will maintain a record of this personal information at the COPPERSTATE office so that we will be able to continue to provide to you, the policy holder, services.
Each of the employees of COPPERSTATE are required to sign and acknowledge all of the provisions of this Privacy Notice. Each employee has been instructed to maintain the privacy of the personal information received from any of our customers and the importance of protecting all of this information on our customer's behalf.
If you use your credit card to pay the premium, your credit card information is deleted from COPPERSTATE's records after your purchase has been authorized. A transaction number will be provided to you concerning the transaction on your binder of insurance or elsewhere. You may also use this transaction number as a reference to the credit card purchase. COPPERSTATE does not keep a record of your credit card information. If you pay COPPERSTATE a premium or other amount using your personal check, COPPERSTATE will keep a copy of your check in your file if the transaction requires a file be kept. When you send COPPERSTATE an email to Our Web Site, this will provide us with certain personal and identifiable information including your email address, which is personal information.
2. Cookies. A cookie is a piece of data that is stored on a visitor's hard drive while they are visiting a Web site. COPPERSTATE may utilize a short form of cookie when you visit Our Web Site. At COPPERSTATE, a cookie is only used to identify one unique visitor from another visitor during a particular session.
